Overview

You can correct your passport if there is a data or printing error. A data error may include an issue with your name, sex, or place of birth. A printing error may include data missing on the biographical page, discoloration, or crooked printing. We will correct the error at no charge if your passport is still valid.

Applications for data correction or printing errors must be submitted by MAIL or DROPBOX.

IMPORTANT! If you are reporting the error within one year of us issuing your incorrect passport, the new passport will be valid for 10 years. If you are reporting the error after one year, the new passport will be valid until the expiration date of your original, incorrect passport.

Required documents/actions

  • Passport Application. Complete online, print on single-sided paper, hand-sign (in black ink) and date the DS-5504 form. Applications submitted without your signature and date will not be processed. For applicants that have never been issued a U.S. Social Security Number, please fill out this affidavit (PDF 97K).
  • Current Passport: Submit your most recent U.S. passport.
  • Evidence of correct biographic information (e.g. your U.S. birth certificate showing the correct spelling of your last name). 
  • No Fee
    • Return of passports: A DHL label is required. PRINT out the label and INCLUDE it in the application package you send to the Embassy/Consulate. In-person pick up is not available.

  • If you were issued a SSN and do not remember it, please contact the Federal Benefits Unit at the U.S. Embassy in Madrid.
  • If you have never been assigned a SSN, complete a statement to that effect (under penalty of perjury), and submit it along with the passport application.

  • Passport processing times vary from 4-6 weeks. If you have travel plans, indicate the dates on your application.
  • You may contact us: through the Passport Assistant ONLY under the following circumstances:
    • If you have urgent travel while your application is being processed, (you must present a travel itinerary) OR,
    • You sent us your application more than 6 weeks ago.
