Overview

If you have changed your name because of a recent marriage, divorce, or a legal name change, you can apply for a new passport in your new name.

  • Name change occurred more than one yearsince issuance of most recent passport.
    Select the option that applies to you:

    • Renew by MAIL only if you meet ALL the following requirements:
      1. You can submit your most recent passport.
      2. You were at least 16 years old when the passport was issued.
      3. Your passport was issued less than 15 years ago.
      4. Your passport is not damaged or mutilated.
      5. You can present a name change document (marriage license, divorce decree, or legal name change document) reflecting your new legal name.

Renew by IN-PERSON APPOINTMENT: If you cannot present a name change document (marriage license, divorce decree, or legal name change document) reflecting your new legal name OR if you are applying for a MINOR OR FIRST ADULT PASSPORT, you must apply in person. Please contact us through our U.S. Passport Assistant under Change Name or Correct an error section.

Required Documents/Actions

  • Passport Application. Complete online, print on single-sided paper, hand-sign (using black ink) and date the form. Applications submitted without your signature and date will not be processed. For applicants that have never been issued a U.S. Social Security Number, please fill out this affidavit.
  • Current Passport: Submit your most recent U.S. passport. 
  • Original name change document: Include the original marriage license, divorce decree, or legal name change document. It MUST reflect the new legal name.
  • Fee:
    • Name change occurred less than one year ago NO FEE is required.
    • Name Change occurred more than 1 yr. PASSPORT FEE is required. Pay online through the U.S. government’s secure payment site. PRINT out the payment confirmation and SEND IT in the package with your application.
  • Return of passports: A DHL label is required. PRINTout the label and INCLUDE it in your application package sent to the Embassy/Consulate. In-person pick up is not available.

  • If you were issued a SSN and do not remember it, please contact the Federal Benefits Unit at the U.S. Embassy in Madrid.
  • If you have never been assigned a SSN, complete a statement to that effect (under penalty of perjury), and submit it along with the passport application.

  • Passport processing times vary from 4-6 weeks. If you have travel plans, indicate the dates on your application.
  • You may contact us: through the Passport Assistant ONLY under the following circumstances:
    • If you have urgent travel while your application is being processed, (you must present a travel itinerary) OR,
    • You sent us your application more than 6 weeks ago.
