Important Notes for Passport Applicants:

Please use the “form filler” version, NOT the PDF. When you print it, there should be a QR bar code in the upper left-hand corner of the first page. 

In the interest of providing good customer service to all, failure to bring:

• Two (2) pre-cut, 2×2 (5 cm x 5 cm) passport photos  for each applicant, and
• Photocopies of all required documents for each applicant (including a copy of passport bio page or other ID with signature for each passport/ID presented)

Required documentation can be found below and on state.gov (varies by applicant’s age, passport application used, and disposition of previous passport). 

Age Progression Photos:  If the identity of the applicant cannot be established by the photograph in the U.S. passport, pictures that show the progressive growth of the applicant should be submitted at the time of interview.  Failure to comply with this requisite may delay in the passport application process.

Any foreign language document must be accompanied by a certified English translation.

Passports generally take about four weeks to arrive. You will be notified by email when your new passport arrives.  Bring your old passport with you to pick up the new passport (unless the old passport is expired), so that we can cancel the old passport.
