The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances. In Nigeria, the DS-3053 can only be notarized at the U.S. Consulate in Lagos or the U.S. Embassy in Abuja, by appointment. To book an appointment, visit ng.usembassy.gov/services/notarials

You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
If you are travelling abroad and your passport validity expires, you can apply for a full-validity regular passport or a limited-validity emergency passport. Replacing a full-validity passport requires a minimum of two weeks. An emergency passport can usually be issued within two business days. Note: All fees are non-refundable. The passport fee is a non-refundable processing fee and is retained by the Department of State whether or not the passport is issued.
