Schedule an Appointment

When attending your American Citizen Services appointment at the U.S. Embassy, you may be accompanied by an individual providing assistance—such as an attorney, interpreter, caregiver, or family member. Please ensure you notify us of their identity in advance by emailing LjubljanaACS@state.gov and including “E99” in the subject line. All visitors to the embassy are subject to standard security procedures and screening.

  • The presence of an attorney or other third party does not waive the requirement for the applicant or the applicant’s parent/legal guardian to appear in person, as applicable.

  • Any costs associated with third-party attendance are the sole responsibility of the applicant.

  • Attendees may take written notes during the appointment but are not permitted to otherwise record the proceedings.

If you wish to authorize the U.S. Embassy or Consulate to discuss your case with a third party, you will need to complete a Privacy Act written consent form, which will be available at the time of your appointment.

Overview

Notarial services are available to individuals of all nationalities and are provided by appointment only. These services are typically intended for documents to be used in the United States, though exceptions may apply. If you require notarization of multiple documents, please schedule only one appointment. A fee of $50 USD (pay by Euros cash or Credit card) per notary seal will be collected at the Embassy or Consulate on the day of your appointment.

On the day of your appointment, you must:

  • Bring the complete, unsigned documents to be notarized. The entire document packet must be presented, including any pages that do not require a signature or seal.

  • Present a valid government-issued photo ID, such as a passport or driver’s license. The name on the documents must match the name on your ID.

  • Pay $50 USD (pay by Euros cash or Credit card) for each notary seal.

  • Be of sound mind and capable of understanding the document. Consular staff are not authorized to explain document contents.

  • If your document requires witnesses, you must provide your own. Consular staff cannot serve as witnesses.

Examples of Notarial Services Performed At No Charge

DS-3053: To notarize a DS-3053 Statement of Consent: Issuance of a U.S. Passport To a Minor Under Age 16 (PDF, 345K), please review the instructions listed on the form, the information fields that must be completed, and bring your original, valid, government-issued photo ID as well as a photocopy of both sides. As the U.S. Department of State requires that this form be notarized, this service is performed free of charge.

Power of Attorney (in conjunction with U.S. passport applications): When both parents are unable to be personally present to apply for a minor’s U.S. passport, and they wish to designate a third party to do so, they may sign a power of attorney (POA) before a notary public. This POA must contain specific data fields; see a sample.  Note that photocopies of both sides of each parents’ original, valid, government-issued photo ID must be included with the POA. As the U.S. Department of State requires that this form be notarized, this service is performed free of charge.
